Medical Leave of Absence Letter

Medical leave of absence letter is a letter made by an employee to his or her employer informing of his or her medical condition and request for a medical leave for a duration of period as advised by the attending physician. This letter will serve as a documentation together with the information related to health condition that will be submitted to the company for record and verification.

Re. Medical leave of absence

I am writing this letter to request a medical leave for a duration of 1 month and 15 days starting the 1st of June of 2015. I would like to inform you that I am recently diagnosed of breast cancer stage 1. I was advised by my surgeon doctor to undergo a total mastectomy on my right breast which is scheduled on the 1st of June 2015. After the operation, I need to have a complete bed rest for fastest recovery and I will referred to one of the Oncologist to examine me what options will be the best treatment for my cancer.

I expect to return for work by the 15th of July 2015. As I come back for work, I will submit all the documents related to my surgery, confinement and treatment for my health condition. In case, I cannot return on the specified date, I assure you that I will notify you as soon as possible. Thank you
