Re. Medical leave of absence
I am writing to request you medical leave of absence for a period of one month starting 30th July 2009. As you are aware, I have been diagnosed as having a cyst in my uterus by the gynecologist and the only remedy for the same, is surgery, which is scheduled for the 31st of July 2009. Post surgery, I have been advised complete rest for a period of one month for total recuperation.
I expect to return to work by 1st August 2009 and resume my duties. I shall submit all documentation regarding the surgery as may be required. In case, I cannot resume office on the specified date, I assure you that I shall provide prior notification.
