Maternity leave letter to boss is a letter given by a pregnant woman who is employed to her current company or institution informing the authority and the supervisor for her critical situation in pregnancy stage that needs to have a complete bed rest or near due date of giving birth to her baby that requires filing of maternity leave as requested from her obstetrician doctor.

I am writing this letter to inform you of my near due date of giving birth to my baby boy hoping to be in normal delivery and based from the ultrasound and my doctor’s computation for the date of delivery it is expected on May 12, 2015. My doctor advised me to have my early admission before the expected date for close monitoring and observation which is on the 8th of May. Therefore, I need to file my maternity leave starting from my admission. I am aware that during my leave I will receive a salary and compensatory benefits from our company.
After giving birth I will inform right away our office especially your department when can I come back for work with the advised of my doctor and if there is a need to extend my leave I will inform you as well.
I hope you will grant my maternity leave request as soon as possible and notify me. Thank you.
