The opening paragraph of the letter must mention the name of the parents and the child/children for whom the consent is given. The paragraph must also mention the reason for which the consent is provided.
The next paragraph must mention other details of the task or the activity for which the consent is given. The paragraph can also mention any condition if imposed by the parents before giving their consent.
The last paragraph of the letter is used by the parents to mention about any enclosed documents or any other additional information that needs to be conveyed regarding their children.
