Consent Letter For Child Travel

A consent letter for child travel may be necessary in several occasions. If the child is travelling with a group from school within the country, or is he/she is travelling with one parent while the parents are divorced, or even if the child is travelling with a relative (not a parent) then this consent letter is necessary.

Dear Sir/Ma’am

Subject:  Consent letter for child travel

I, Mary Watson, mother of Shirley Watson (age 10) do hereby authorize my daughter to travel with her maternal aunt (my sister) Elizabeth Holmes from New Jersey to San Francisco.

My husband James Watson will be out of the country for a period of one month on business and I will also be travelling extensively during this time. Hence I authorize my sister Elizabeth to act guardian to my daughter. She will be free to accompany my daughter to the places that they need to travel during that one month period starting 08.02.2014 through 09.01.2014.

I further authorize Elizabeth to make medical decisions for Shirley during this period. Elizabeth will need to accompany Shirley back to New Jersey on 09.01.2014 where either I or my husband James Watson will take over her custody. Shirley is however not to be left alone or allowed to travel without a guardian at any point. I will be available on the phone at 2402916788 throughout the day; so please feel free to call me in case of any query or emergency.
